Clayton Cook was born and raised in Cache Valley, Utah, and comes from a large family – he is
the 5th of 6 children. He attended Mountain Crest High school in Hyrum, Utah while he was in
high school he was actually a state champion in Football. In high school Dr. Cook met his best
friend, Kallee, who is now his wife. After graduating from high school, he worked in the Dallas
area for a short time and fell in love with Texas. He left Texas to serve a 2 year Christian mission
in Hiroshima, Japan. Shortly after returning, he married his wife, and they’ve been married for
20 years and have 6 beautiful children. Dr. Cook graduated from Utah State University, majoring
in Biology with a minor in Chemistry and Japanese. He attended dental school at Midwestern
University in Glendale, Arizona. Dr. Cook pursued dentistry because he enjoys working with
people and taking care of their needs in an empathetic and compassionate manner. Dentistry is
a natural fit for him and it is a great balance of hard work and making a real difference in
people’s lives. In his spare time, Dr. Cook enjoys hiking, playing sports of all kinds, specifically

basketball which has been his favorite since he was 5, reading, surfing which he did with his son
Jared, and a recent hobby of his is downhill mountain biking. Dr. Cook enjoys traveling and a
few of his favorite trips are going snow skiing in Park City, Telluride, or Taos. Overall, Dr. Cook is
always up for an adventure and enjoys spending time with his family. He enjoys living in Texas
and getting to work in our Fort Worth and Glen Rose offices and making a change in people’s
lives.
Dr. Krystal Moore attended Texas A&M/Baylor College of Dentistry. She is the mother of three beautiful children that are her biggest blessings and greatest cheerleaders. Her core set of values are centered around honesty, kindness, and compassion.
Dr. Moore deeply respects all patients and employees and is known for creating a positive environment at Polished Family Dental. Dr. Moore enjoys communicating in Spanish for her Spanish-speaking patients. She loves working with adults and children and educating her patients on healthy habits!
Dr. Moore is also passionate about photography and dance and uses her artistic side to create beautiful smiles.
Jeremy Pope was born and raised just south of Fort Worth in Itasca, Texas. Dr. Pope knew from the age of five that he wanted to be a dentist! Between playing pee-wee football and baseball, he would build dental offices and read the medical dictionary to practice for his future career. He told his mom he just wanted to help people smile.
Dr. Pope graduated from The University of Texas at Arlington with a major in Biology and received his DDS from The University of Texas School of Dentistry at Houston.
Dr. Pope met his best friend and wife, Mollie, while playing ultimate frisbee in 2010 at her alma mater, TCU. Go Frogs!
They were married in 2016 in downtown Fort Worth and have two wonderful children, Jack and Ella. When he’s not practicing dentistry, Dr. Pope enjoys spending time with his beautiful wife, making his kids laugh, leather crafting, watching sports, and working out.
Fort Worth is home to Dr. Pope, and he is so happy to be part of the Polished Family Dental team!
Dr. Glen Petta grew up in Fort Worth and attended Paschal High School where he played basketball and football. He completed his undergraduate education at Texas Christian University on an academic scholarship and attended Baylor Dental School in Dallas, graduating in 1980. During his time in dental school, Dr. Petta opened and managed a Dallas restaurant that further expanded to four locations. He continued in the restaurant business for 25 years while actively practicing dentistry.
Dr. Petta returned to Fort Worth and bought into the practice of William F. Wathen, DMD, where he practiced for 26 years. The office opened a satellite office in Aledo, which they later sold.
Dr. Petta left his practice in 2010 to lead a Christian international mission agency for 12 years. He led and trained over 150 mission teams all over the world. During that time, he continued to practice part time while not on missions. He has been very active in international missions. He is a life member of the American Dental Society, Texas Dental Society, and Fort Worth District Dental Society.
Dr. Petta and his wife of 46 years, Debbie, have two sons. Andrew, a priest, is married to Michelle, and they have two daughters, Chloe and Claire. Philip works in finance and accounting for a rail company.
Cheyenne has been an invaluable part of our team for over 10 amazing years! Her passion for dentistry ignited when she became a Registered Dental Assistant in 2013, and since then, she’s dedicated herself to making a difference in patients' lives. Cheyenne truly loves helping patients navigate their dental insurance, arranging finances, and most importantly, finding creative solutions to give everyone the smile of their dreams!
When she’s not brightening up the office, Cheyenne spends her time with her wonderful husband and their two energetic daughters, ages 9 and 13. Her weekends are filled with adventure—whether she’s camping, kayaking, or enjoying time with family and friends, Cheyenne’s zest for life shines through.
Oh, and don’t forget about her beloved animals! She’s a true nature lover with a heart of gold.
Born and raised in Fayetteville, Georgia, Sarah has been a dedicated phlebotomist since 2022, bringing both precision and compassion to her work. In addition to her medical expertise, she serves as our VA Coordinator, ensuring that veterans receive the highest quality healthcare and support they deserve.
Outside of work, Sarah is just as driven and passionate. A former Junior Olympian and NCAA athlete, she has excelled in both swimming and throwing events. She enjoys staying active, spending time outdoors fishing, and unwinding with her dog. She also loves expressing her creativity through singing.
Sonia was born and raised in Fort Worth. She knew she wanted to get into dentistry to make people feel confident with their smile. Sonia loves answering questions for patients and teaching about prevention. She went to school at Tarrant County College, graduated in 2010, and has been practicing for 15 years.
In her spare time she enjoys spending time with her husband of 15 years and their two daughters. They love staying busy, swimming, dancing, spending time with family and friends, and being at their daughters’ soccer games.
Margarita was born in Decatur, TX, and raised in Bridgeport as the youngest of ten siblings. In 2020, she made the move to Fort Worth and quickly fell in love with the city and its vibrant community. She has been a practicing dental hygienist since 2017 and finds it incredibly rewarding to help patients improve their oral health habits. Building trust and seeing positive changes in her patients’ smiles is what drives her passion for dental care.
Outside of the office, Margarita enjoys spending time outdoors—whether it’s hiking or biking along the Fort Worth Trinity Trails. She also loves movies, collecting nature photography, and exploring new restaurants around town.
Kristina has been a dedicated dental hygienist for over 10 years, and her passion for patient care continues to shine through in every appointment. She loves building strong, lasting relationships with her patients and takes pride in helping them achieve healthy, confident smiles.
When she's not in the office, Kristina enjoys traveling and exploring new places—but her heart is always with her hobby farm, where she cares for goats, chickens, and even a tiny pig! Her warm personality and gentle approach make every visit a positive experience.
Hi, I’m Cindy! I’ve been a dental assistant for 13 years, and my passion for dentistry started at a young age. Inspired by my mom, a former dental assistant, I saw firsthand the impact a caring dental team can have. Following in her footsteps, I discovered my own love for the dental field—from assisting in clinical care to helping create healthy, confident smiles.
What I enjoy most is connecting with patients and being a part of their journey toward better oral health. It’s incredibly rewarding to make a difference, even in the smallest ways, that can brighten someone’s day—and their smile.
As a passionate dental assistant since 2015, Ashley is proud to be a part of a fantastic team dedicated to providing top-notch dental care. After enjoying six years in Granbury, she will soon be relocating back to Fort Worth where she was born and raised.
When not assisting, she spends quality time with her four children, who keep her on her toes and busy with dance and sports activities. When she has a moment to herself, she’ll binge-watch the latest Netflix series.
Erick is a part of our team and loves getting to work here. He is from Monterey, Mexico. He lived in Houston for 10 years, where he attended high school, and moved to Fort Worth about five years ago. He found his passion for dentistry back in 2006 when he moved back and started dental school at UANL in Monterey.
After finishing school, he came back to the United States and started working as a sous chef for seven years and is now in the dental field again. His goal is to become a dental hygienist. Erick loves the dental field and is so glad to be back.
Born in Corpus Christi and raised in Kansas, Angie’s journey has been nothing short of an adventure. After nearly two decades away, she returned to Texas in 2021, settling in the vibrant city of Fort Worth. While her career initially began in nursing, fate had other plans. Angie’s bilingual skills opened the door to the world of dentistry, and she quickly discovered her true passion. With over 22 years of experience in the dental field, Angie has truly found her calling, making a lasting impact on everyone she serves.
Outside of work, Angie loves spending time with her husband, children, and grandchildren, who keep her grounded and filled with joy.
Westin oversees marketing for the practice, using her creative eye to bring fresh ideas and engaging content to our team. She graduated from Tarleton State University in 2022 and went on to earn her master’s degree in Management in Fall 2024, making her a proud Tarleton Alum x2. She has been with the practice for almost two years and has loved every moment of building relationships with patients, finding inspiration in their stories and the connections she makes along the way.
Outside the office, Westin enjoys spending time with friends and family, reading, cooking, and embracing the simplicity of life in her camper. She was recently engaged to her fiancé, Jacob, and is enjoying every moment of this exciting new chapter. She’s always on the lookout for a great book, so if you have any recommendations, be sure to send them her way!
