Clayton Cook was born and raised in Cache Valley, Utah, and comes from a large family – he is the 5th of 6 children. Growing up, Dr. Cook spent many hours in his father’s podiatry office cleaning rooms, stocking cupboards, and – when his dad wasn’t looking – playing with equipment. Consequently, Dr. Cook discovered his fascination with the medical world at a very young age.
Dr. Cook met his best friend, Kallee, now his wife, in 9th grade. After graduating from high school, he worked in the Dallas area for a short time and fell in love with Texas. He left Texas to serve a Christian mission in Hiroshima, Japan. Shortly after returning, he married his wife, and they’ve been married for 8 years and have 4 beautiful children.
Dr. Cook graduated from Utah State University, majoring in Biology with minors in Chemistry and Japanese. He attended dental school at Midwestern University in Glendale, Arizona.
In his spare time, Dr. Cook enjoys hiking, playing sports of all kinds, reading, challenging his wife to a game of scrabble, and giving his kids “Wild Buckin’ Bronco” rides in the family’s front room. He’s always wanted to make it back to Texas and is so excited to be in Fort Worth.
Dr. Krystal Moore is the mother of 3 beautiful children that are her biggest blessings and greatest cheerleaders. Her core set of values are centered around honesty, kindness, and compassion. Dr. Moore deeply respects all patients and employees and is known for creating a positive environment at Polished Family Dental. Dr. Moore speaks both English and Spanish. She loves working with adults and children and educating her patients on healthy habits! Dr. Moore is also passionate about photography and dance and using her artistic side to create beautiful smiles!
Jeremy Pope was born and raised just south of Fort Worth in Itasca, Texas. Dr. Pope knew from the age of five that he wanted to be a dentist! Between playing pee-wee football and baseball, he would build dental offices and read the medical dictionary to practice for his future career. He

Dr. Pope graduated from The University of Texas at Arlington with a major in Biology and received his DDS from The University of Texas School of Dentistry at Houston.
Dr. Pope met his best friend and wife, Mollie, while playing ultimate frisbee in 2010 at her alma mater, TCU. Go Frogs! They were married in 2016 in downtown Fort Worth and have two wonderful children, Jack and Ella. When he’s not practicing dentistry,
Dr. Pope enjoys spending time with his beautiful wife, making his kids laugh, playing and building guitars, watching football, and working out. Fort Worth is home to Dr. Pope, and he is so happy to be part of the Polished Family Dental team!
Dr. Glen Petta grew up in Fort Worth and attended Paschal High School where he played basketball and football. He completed his undergraduate education at Texas Christian University on an academic scholarship and attended Baylor Dental School in Dallas, graduating in 1980. During his time in Dental School, Dr. Petta opened and managed a Dallas restaurant that further expanded to four locations. He continued in the restaurant business for 25 years while actively practicing dentistry.
Dr. Petta returned to Fort Worth and bought into the practice of William F. Wathen, DMD, where he practiced for 26 years. The office opened a satellite office in Aledo which they later sold. Dr Petta left his practice in 2010 to lead a Christian international mission agency for 12 years. He lead and trained over 150 mission teams all over the world. During that time, he continued to practice part time while not on missions. He has been very active in International missions. He is a life member of the American Dental Society, Texas Dental Society, and Fort Worth District Dental Society.
Dr Petta and his wife of 44 years, Debbie, have two sons. Andrew, a priest, is married to Michelle, and they have two daughters, Chloe and Claire. Philip works in finance and accounting for a rail company.
Cheyenne has been with our team for almost 10 years! She has been fascinated with dentistry since becoming a registered dental assistant in 2013. She found a love for helping patients with their dental insurance, financial arrangements and creating solutions to give patients their dream smile! Cheyenne enjoys spending time with her husband and two daughters, 7 and 11 years old. While away from the office you can find her camping, kayaking, tending to her animals and spending time with family and friends.
Amy has been with our team since 2017 and has over 20 years experience in the dental field. In addition to her dental assistant responsibilities, she also oversees our inventory and orders supplies. Amy loves building relationships with patients and helping people overcome their fears. In her free time Amy loves spending time with all her grandchildren.
Sonia was born and raised in Fort Worth. She knew she wanted to get into dentistry to make people feel confident with their smile. Sonia loves answering questions for patients and teaching about prevention. She went to school at Tarrant County College, graduated in 2010, and has been practicing for 13 years. In her spare time she enjoys spending time with her husband of 13 years and their 2 daughters. They love staying busy, swimming, dancing, and spending time with family and friends.
Angie was born in Corpus Christi and moved to Kansas shortly after where she was raised. It was not until 2021 that Angie returned back to Texas and moved to Ft. Worth. Angie did not originally start in dentistry she was in nursing however, she was offered a job in dentistry because she is bilingual. She has been in the dental field for over 22 years and this is truly where she has found her passion in her life. When she is not working Angie is spending time with her children and grandchildren.
Erick is a great addition to our team and loves getting to work here. He is from Monterey, Mexico, and lived in Houston for 10 years where he attended high school and moved to Fort Worth about 5 years ago. He found his passion for dentistry back in 2006 when he moved back and started dental school at UANL in Monterey. After finishing school he came back to the United States and he started working as a sous’ chef for 7 years and is now in the dental field again. His goal is to become a dental hygienist. Erick loves the dental field and is so glad to be back.
Westin is new to our team and has recently joined to oversee our marketing for the practice. She loves getting to have a creative eye and getting to share her ideas with everyone. She graduated from Tarleton State University in 2022 and is currently working on her masters there too. She is set to graduate in the Fall of 2024 with a degree in Management and cannot wait to be a Tarleton Alum x 2. When Westin is not in the office you can find her spending time with friends and family, reading, cooking, or enjoying living in her camper! If you ever have any book recommendations send them her way!
Ashley was born and raised in Fort Worth, Texas, and relocated to Granbury in 2019, seeking a more relaxed pace away from the hustle and bustle of city life. She has been a dedicated Registered Dental Assistant (RDA) since 2015, with a passion for helping patients achieve and maintain optimal oral health. When not assisting in dental care, Ashley enjoys spending quality time with her four children, who keep her busy with their involvement in sports and dance.
